Transforming Texas Grant
The Angelina County & Cities Health District continues its long-standing dedication to improving the health and wellness of all Angelina County citizens as an awardee of the Transforming Texas Grant.
Managed and administered by the Texas Department of State Health Services, Transforming Texas supports and enables communities to identify their leading health problems and design programs that work for them. The program gives communities the opportunity to develop and implement initiatives to prevent chronic diseases, the leading causes of death and disability.
The Transforming Texas program‘s goal is to create healthier communities by making healthy living easier and more assessable where people work, live, learn, and play. The program will contribute to improved health of the county by focusing on improving weight, nutrition, physical activity, and decreased tobacco use of its citizens.
The Transforming Texas initiative is funded by the U.S. Department of Health and Human Services (HHS) Community Transformation Grant (CTG)
